Health-e-MedRecord and CORHIO Partner to Implement Colorado-Wide Advance Care Planning Platform to Better Communicate Patient Wishes

DENVER – October 4, 2018 – Health-e-MedRecord (HEMR), a patient-centered, physician-designed solution to enhance electronic health records, and CORHIO, a leading health information exchange in Colorado, announced today they will partner to implement HEMR’s Medcordance Advance Care Planning platform to ensure patient’s treatment wishes are honored during emergencies.

During a medical crisis, patients could be unable to discuss their treatment options, often leaving family and physicians to make these critical decisions on behalf of the patient. When patients are well, they are urged to make — and document — these decisions.

This partnership with CORHIO will allow providers across the delivery continuum in Colorado to securely share a patient’s MOST form, which is Colorado’s version of the POLST form created by National POLST (www.polst.org). Medcordance allows secure sharing of POLST, Advance Health Care Directives, and other documents during transitions of care, regardless of which electronic medical record system a hospital, clinician, or ambulance provider uses, and allows sharing of documents among family members and caregivers, at the request of the patient.

Health-e-MedRecord’s proprietary MedcordanceTM platform is the first of its kind in the country to integrate Advance Care Planning clinical workflows with its own Qualified Clinical Data Registry certified by the Center for Medicare and Medicaid Services to report electronic submissions of Physician’s Orders for Life-Sustaining Treatment (POLST) into the Medcordance ePOLST Registry under the provider Merit-based Incentive Payment System.
