WE ARE HEALTH-E-MEDRECORD
Designed by physicians who work in all areas of medicine (inpatient, outpatient, emergency department, surgery, administrative), Health-e-MedRecord places the patient at the center of a treatment team that can efficiently create and electronically share treatment plans, medication lists, outpatient clinical notes, and advance care planning documents, anywhere and anytime.
Health-e-medrecord.com is not just a technology company. Health-e-MedRecord is proudly owned by practicing clinicians who were frustrated that existing health technology doesn’t make critical information readily available to providers that profoundly impacts patient care. HEMR’s philosophy is to build clinician-designed cloud-based technology that maximizes physician adoption and provides meaningful patient engagement. HEMR’s flagship product, Medcordance, is the manifestation of our commitment to align with the vision of the California Coalition for Compassionate Care to further POLST.
Health-e-MedRecord is
QCDR Certified
On December 29, 2017, HealtheMedRecord (HEMR) became certified by the Center of Medicare and Medicaid Services as the first Qualified Clinical Data Registry (QCDR) for electronic POLST. HEMR has partnered with the Ventura County Medical Association to launch the nation’s first QCDR electronic POLST Registry powered by Medcordance. Starting in 2018, physicians can report electronic POLST submissions as a quality measure under the Merit-based Incentive Payment System (MIPS), in order to avoid Medicare’s payment penalties for failure of physician reporting under MIPS.
Having patients’ Advance Care Planning wishes available to providers in the CORHIO network will improve clinical decision-making, enhance participation in accountable care and value-based payment models, and take this difficult burden off of family members in a time of crisis.”Morgan Honea, CEO of CORHIO
